Documents to apply for certification.
Within the specified deadlines, submit in writing to Ekoagros the documents in the forms established, according to the areas to be certified:
- Those requesting to certify the primary production of crop and livestock production before the end of the crop declaration for the current year have to submit:
Application for organic certification, form (F-002 EN);
Initial assessment form of organic production holding, form (F-003 EN);
In accordance with the Rules for the application for support for agricultural land and other areas and livestock and administration and control of direct payments, the applicants have to submit an Application for support for agricultural land and other areas and livestock, the data of which will be automatically transferred to Ekoagros information system. Holdings which intend to certify livestock in the current year, when completing the Application for support for agricultural land and other areas and livestock, must indicate the type of livestock to be certified in the dedicated section in the Application Information System.
- Those requesting to certify the primary production of apiculture before the end of crop declaration for the current year have to submit:
Application for organic certification, form (F-002 EN);
Initial assessment form of organic apiculture holding, form (F-014 EN).
- Those requesting to certify the primary production of aquaculture before the end of crop declaration for the current year have to submit:
Application for organic certification, form (F-002 EN);
Initial assessment form of organic aquaculture holding, form (F-035 EN);
Organic aquaculture production program (plan), attachment of form (F-035 EN).
- Those requesting to certify the wild plant collection the whole year can submit:
Application for organic certification, form (F-002 EN);
Wild plants collection form, form (F-030 EN);
Report on the application of synthetic materials, form (F-030-1 EN).
- Those requesting to certify the processing of organic production for the first time, have to submit:
Application for organic certification, form (F-002 EN);
Form for certification of processing of organic production, form (F-008 EN) and the relevant documents, indicated at the end of this form:
- A copy of the registration fee payment (provided in the section “FEES FOR CERTIFICATION WORKS”);
- Formula of food, requested to certify, form (F-008/1);
- Formula of feed, requested to certify, form (F-008/3);
- A copy of the documentary evidence of the supplier of raw materials / products certified outside Lithuania and a translation into Lithuanian (the translation may be carried out and approved by the legal entity itself, i.e. there is no need for official translation of translation agency);
- A statement of the vendor that the production sold were not produced from genetically modified organisms (GMOs) or using GMO, form (F-102 EN) if non-organic raw materials are being used;
- Drafts of original labels in two copies for the production requested to certify for coordination, before printing;
- Control procedure for organic raw materials / production (requirements of control procedures) with indicated critical points and preventive measures (in the procedure provide information about organic / in-conversion production / raw materials acceptance, storage, separation during production / storage, labelling, traceability, marketing, withdrawal from the market, complaint registration system);
- Plans for all warehouses and auxiliary premises with a marked path for the movement of organic products;
- A copy of the contract with the subcontractor if some or all of the production activities will be carried out by another entity. The contract must include a clause: to allow the representative of PI Ekoagros to inspect in detail the entity, the documents related to the entity and the activities of the client;
- A valid certificate of food handling entity approval / copy from the register of the State Food Veterinary Service.
